NHS 111

In the case of urgent need when the practice is closed you can call NHS 111. Your needs will be assessed and advice offered or arrangements made for you to see a doctor.

Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery re-opens.

Call 999 in an emergency. Chest pains and / or shortness of breath constitute an emergency.

Patient Access

How do I Register?

You can either register by visiting your surgery and completing an application form and a receptionist will verify your identification (please ensure you bring a form of photo ID).

Once you have completed your registration you will then be able to book routine GP appointments and manage your existing appointments.

For more information please visit our appointment page.

Access to Medical Record

You can also apply for access to your records by completing a form available at reception. Please allow four weeks after the registration is completed for access to be granted.

Proxy Access

A new feature has been recently introduced which allows patients to give access to a family member, carer or family member to order repeat medication and book routine appointments on their behalf.  If you would like someone to be nominated as your proxy you will both need to attend the surgery with a form of photo ID and complete a registration form.

Things to consider prior to registering:

Misunderstood Information

Your medical record is designed to be used by clinical professionals to ensure that you receive the best possible care. Some of the information within your medical record maybe highly technical, written by specialists and not easily understood. If you require further clarification, please contact the surgery so we can arrange a telephone appointment with the Doctor.

Choosing to share your information with someone

It's up to you whether or not you share your information with others-perhaps family members or carers. It is your choice, but also your responsibility to keep the information safe and secure.

If you think you maybe pressured into revealing details from your patient access to someone else against your will, it is best you do not register at this time.

Forgotten History

There maybe something you have forgotten about in your record that you may find upsetting.

Abnormal Results/Bad News

If your GP has given you access to test results or letters, you may see something that you find upsetting. This may occur before you have spoken to the Doctor or the surgery is closed and you cannot contact them.

Information about someone else

If you spot something in your record that is not about you or you notice other errors, please log out of the system immediately and contact the practice.
